What Should I Do To Prepare For My Appointment?
- Please call our office at (219) 525-4815 to verify your insurance benefits beforehand.
- Have in writing key past medical history, previous treatments, current medications, and other important information.
- List out signs and symptoms that can be of importance to the therapist. What has been hurting or bothering you? For how long?
- It is a good idea to make a list of questions that you want to discuss on your first visit.
- Please arrive 15 minutes early for your first appointment.
- Bring a copy of your insurance card and a photo ID with you.

Let’s face it, insurance information can be quite the doozy to figure out on your own, especially when you are trying to decipher whether or not your physical therapy treatment is covered under your policy. At Apex Physical Therapy, we accept a variety of different health insurance plans, and we are here to help you navigate the world of claims and benefits.
We understand that insurance can be complicated, so as a courtesy, we will check your health benefits before you arrive to see if there will be a copay, deductible, or coinsurance for treatment services.

How Does Payment Work?
- Your insurance benefits will be determined before your first visit, and you will be informed of what, if any, payment is necessary on your appointment date.
- We accept cash, check, debit card, and credit card. We are happy to discuss payment plan options as well. It is our policy that payment is made at the time of service.
How Should I Prepare For My First Appointment?
When you call to have us schedule your first appointment, please have your prescription (if applicable) and insurance card handy. This will allow us to confirm your insurance benefits and give you a coverage estimate.
To bring/keep in mind on your first visit:
- Bring any case manager or insurance adjuster contact names, phone numbers, and claim numbers in the event of an automobile accident or worker's compensation claim.
- A prescription (if needed) for physical therapy from your referring physician, ARNP, or PA.
- Insurance information (we will make a copy of your card).
- Print out the patient forms, fill them out, and bring them with you. If you are unable to do this, please allow for extra time to fill these out upon your arrival.
- Please arrive 15 minutes early to complete any remaining paperwork, so you can maximize your time in your evaluation.
- Please wear comfortable clothing that allows easy access to your problem areas.
